Children are often more relaxed and cooperative in their own environment, parents can be more actively involved in the session, and the physiotherapist can observe the child's real-world movement and play — which is clinically invaluable. This article explains which conditions can be treated at home, what a paediatric home visit looks like, and when to seek help.
A wide range of paediatric conditions can be effectively treated with home visit physiotherapy. These include: developmental delay (children who are not meeting motor milestones such as sitting, crawling, or walking at the expected age); hypermobility and joint hypermobility syndrome (children with very flexible joints who experience pain, fatigue, or coordination difficulties); sports injuries (sprains, strains, and overuse injuries in active children); postural problems (scoliosis, kyphosis, and poor posture that is causing pain or affecting function); neurological conditions (cerebral palsy, spina bifida, and other conditions affecting movement and coordination); and musculoskeletal pain (growing pains, Osgood-Schlatter disease, and other conditions causing pain in growing children).
A paediatric home visit begins with a conversation with the parents about the child's history, development, and current concerns. The assessment is then conducted in a child-friendly way — often through play and movement activities rather than formal clinical tests. I observe how the child moves, plays, and interacts with their environment. The physical examination is adapted to the child's age and cooperation level. Treatment is similarly adapted — for young children, exercises are disguised as games and play activities to maintain engagement and motivation.
Children are often more relaxed and cooperative in their own home than in a clinical setting. This is particularly important for children with anxiety, autism spectrum disorder, or sensory processing difficulties, who may find clinic environments overwhelming. At home, I can observe the child's actual toys, play equipment, and daily activities — which gives me a much richer picture of their function than a clinic assessment can provide. Parents are also more actively involved in home sessions, which improves their understanding of the programme and their ability to support their child's progress between sessions.
You should seek paediatric physiotherapy assessment if your child: is not meeting motor developmental milestones (ask your health visitor or GP for age-specific guidance); is complaining of persistent joint or muscle pain; has been diagnosed with a neurological condition affecting movement; has had a sports injury that is not resolving; or has a postural problem that is causing pain or concern. Effective paediatric physiotherapy involves the whole family and, where relevant, the child's school.
