Does Online Physiotherapy Actually Work?

Online physiotherapy — assessment and treatment delivered via video call — has grown significantly in recent years. Many patients are sceptical: how can a physiotherapist treat you without being in the room? The answer is more nuanced than you might expect, and for the right conditions, online physiotherapy is genuinely effective.

What Online Physiotherapy Actually Involves

An online physiotherapy session is conducted via video call — using any device with a camera. The physiotherapist assesses your movement, posture, and function through observation and guided tests, asks detailed questions about your symptoms, and then provides a personalised exercise programme, education, and advice. Follow-up sessions review your progress and progress the programme accordingly.

The Evidence for Online Physiotherapy

Research published in the last five years consistently shows that online physiotherapy produces comparable outcomes to in-person treatment for many musculoskeletal conditions — including back pain, neck pain, and knee pain. A 2021 systematic review found no significant difference in pain or function outcomes between telehealth and in-person physiotherapy for musculoskeletal conditions.

When Online Physiotherapy Works Well

Online physiotherapy is particularly effective for conditions where exercise prescription, education, and movement coaching are the primary treatment tools — which covers the majority of musculoskeletal conditions. It's also ideal for follow-up sessions after an initial in-person assessment, for patients who travel frequently, and for those who prefer the convenience of appointments from home.

When In-Person or Home Visit Treatment Is Better

Online physiotherapy is less suitable when hands-on manual therapy is a significant component of treatment — for example, joint mobilisation for severe stiffness, or soft tissue work for acute muscle injuries. For neurological conditions, post-surgical rehabilitation, and elderly patients with complex needs, in-person or home visit physiotherapy is generally more appropriate.

How to Get the Most From an Online Session

For the best results from an online physiotherapy session: ensure you have a clear space to move around, wear comfortable clothing, have your device propped up so the physiotherapist can see your full body during movement tests, and have a pen and paper ready to note down your exercise programme.
