Written by Mamdouh Sarhan, MSc, MCSP, HCPC Reg.Booking a physiotherapy home visit for the first time can feel unfamiliar — you might wonder what the physiotherapist will do, how long it will take, or what you need to prepare. The good news is that it is a straightforward, professional, and often surprisingly thorough experience. This guide walks you through exactly what happens during your first home visit physiotherapy session with Direct Physio Care, from the moment I arrive to the moment I leave.
Very little preparation is needed. You will need a clear space where you can sit, stand, and lie down comfortably — your living room or bedroom works perfectly. Wear comfortable, loose clothing appropriate for the area being treated: shorts for knee or hip problems, a vest or loose top for shoulder or neck issues. Have any relevant medical letters, scan reports, or medication lists available if you have them — but do not worry if you do not. I will gather everything I need during the assessment.
Your session begins with a detailed conversation about your symptoms. I will ask about the nature of your pain or problem — where it is, when it started, what makes it better or worse, and how it is affecting your daily life. I will ask about your medical history, any previous episodes of the same problem, and your goals for treatment. This part of the assessment typically takes 15–20 minutes and is the foundation of everything that follows. Do not rush through it — the more information I have, the more precisely I can target your treatment.
After the subjective assessment, I carry out a physical examination. This involves assessing your posture, movement patterns, range of motion, strength, and specific clinical tests relevant to your condition. For back pain, this might include assessing lumbar flexion and extension, straight leg raise tests, and neurological screening. For knee pain, it might include assessing patellofemoral tracking, meniscal stress tests, and hip strength. The physical examination typically takes 15–20 minutes and gives me the clinical information I need to make a diagnosis and plan your treatment.
After the assessment, I will explain what I have found in clear, plain language. I will tell you what is causing your symptoms, why it has happened, and what the treatment plan will involve. I believe strongly in patient education — understanding your condition is a critical part of recovery. Patients who understand what is happening and why are more motivated, more compliant with their exercise programme, and recover faster. I will also give you a realistic expectation of how long recovery is likely to take.
Treatment begins in the same session — you will not leave your first appointment without having received some form of treatment. Depending on your condition, this might include manual therapy (joint mobilisation, soft tissue release, trigger point work), therapeutic exercise, movement re-education, taping, or a combination of these. The treatment is tailored to what your assessment has revealed — not a generic protocol.
At the end of your session, I will give you a personalised home exercise programme — specific exercises for your condition, demonstrated clearly so you know exactly how to perform them. These exercises are designed to continue your progress between sessions and are a critical part of your recovery. I will also give you advice on activity modification, posture, and any lifestyle factors relevant to your condition. The total session time is typically 60 minutes for an initial assessment and treatment.
It is normal to feel some muscle soreness or fatigue in the 24–48 hours after your first physiotherapy session — this is a normal response to treatment and exercise. If your symptoms significantly worsen or you have any concerns, you can contact me directly via WhatsApp. I will also follow up with you after your first session to check how you are getting on and answer any questions about your exercise programme.
Your first home visit physiotherapy session is a thorough, professional, and treatment-focused appointment. You will leave with a clear diagnosis, an understanding of your condition, a treatment plan, and a personalised exercise programme.
